Power of Attorney – Financial
The Wisconsin Department of Health Services provides a free Financial Power of Attorney form for your convenience. You are not required to use this form and may choose any Financial Power of Attorney document that meets your needs.
We CANNOT assist you in preparing these forms. We only notarize them.

Tow Lot Release Form
Vehicles are often towed following an arrest. If the vehicle belongs to someone who is incarcerated, the tow lot may require written, notarized authorization from the registered owner before allowing another person to retrieve the vehicle.
Many tow lots require the vehicle release or authorization form to be signed and notarized in person. A mobile jail notary can visit the incarcerated vehicle owner to complete the required notarization so an authorized person can proceed with retrieving the vehicle.
